8. Reason you should be accepted:I want a good, stable RPG server that won't die out in a few months like has happened to many servers I've applied for.

10. Additional Info: I'll try to be as active as possible but in certain circumstances I may only be able to play weekends. In my spare time I like drawing, simple programming, playing guitar, and playing Battlefield 2 and Runescape.

I don't recall the server name but It could have been the ban I was talking about. I don't remember when exactly it did happen but It was at least a few months.

Actually, the ban I questioned was from April of 2011 as stated by MC Bans.
