Bug Tombstone bug

This has happened many a times on PvE for me where my tombstone does not fill up all the way and yet drops items in my inventory. I do not know why/how this happened. Thank you for reading.

Only hc.to/perks a certain supporter can have a doublechest filled. Does this happen all the time? and what tier are you?

It usually only puts down one chest, I am tier 4 as well.

I noticed the double death chest dropping only once before. Mostly it is only the single chest dropping, and the other items drop next to it, if it's filled to the brim. This seems to happen on large open areas too, where you expect the double chest to drop, but it doesn't. Maybe I'm wrong, I haven't witnessed it often, since the most of the time I die, I never have many items with me. If there's any way to check it you might do so, but I honestly doubt it to be a bug.
