Bug Proxy error with skills

while warming up pulse, my game crashed giving me some sort of proxy error message. This also happened to @TheMechaHeavy using the same skill

It happends over 1/10 times i use it, when I use bonespear it logs me out the server saying (Proxy) you have been disconected.

Once in a fight a few days ago, I also got a [Proxy] Lost Connection to Server message mid-fight.

I received this problem when using flying kick, proxy disconnection and died.

BurkeyZz just died from this bug when he got hit by a sham totem.

Also, it dupes your items when you die like this.
