Bug Ninja's "Incise" skill not working

When I use Incise, a "-0.5" appears above the mob as if it did 0.5 damage, but it actually does 0 damage. Also, its cooldown appears to be 0s instead of the intended 5s. I haven't tried using it in pvp, but I have always had this happen if I try to use it in pve. Additionally, it does not need to be prepared before using; is this intended?
