Bug Items Disappearing After Death

Today I played with a guy and we both died in a mine, by a creeper explosion, at the same time and place.
I dropped a death chest, he didn't. His stuff was supposed to drop at a location close to the death site, at least at a place where it can drop at all, but that didn't happen either.
I know that the items also spread when they drop if the player carries a lot of items, sometimes even to different layers of the ground.
He didn't drop anything at all. And the thing is, that he is at least supposed to drop the items when he dies, but he didn't. Now I have no idea why, and it was really annoying for him, because he lost a lot of really valuable items...

Here some other things about the scenario:
- He died once before and dropped a death chest at the same place (very close to the spot mentioned above where he died at, it's like 2 blocks away), he opened it and took all items but didn't destroy the chest itself. Regularly a DC doesn't prevent another DC to drop next to it.

- He maybe died at exactly the same spot where I died at and my death chest dropped first so it prevented his death chest to spawn or his items to drop (probably)

- I checked several layers (it happened in a mine) and paths, his items were nowhere to find.

- I don't know about creeper explosion physics here, but I do know that they go through blocks even though they cannot break them. It could be that the items were destroyed because there was no reason for them to despawn or any other reason for them to disappear.

- There was no lava or fire around, above or below the place he died at.

- The items did not drop above the ground (above the mine)
