Bug Irovy City death chest bug

I have died twice in ivory city once today and once last week, both times i have died and my death chest have not spawned. This last time I died at -1255 55 -1078 in the boss area and it did not spawn a chest and all my items disappeared but my blessed axe. the chest should be in the logs. thanks

yeah it has not happened to me before, other then those two times. it also happened to smilytyly at the same time i died so idk what is going on but that is the second time i lost some good items to my death chest that never showed up or popped
