Bug Disarm

When you get disarmed as you die you lose your weapon (normally what you are holding). The weapon does not drop, it gets destroyed.

I personally have lost countless weapons to this bug

yeah its annoying. wish they could just change disarm to a debuff that makes you do 1 dmg per hit for 3-4 seconds or whatever.

That would make sense but you know it'll be forever until it's actually fixed.
