the wiki and my skills say that blackjack does over 90 dmg when i sneak, so i shift just before i use it, but on mobs, it shows less hp missing than after i use a kick, which is only around 50.

Bug Blackjack + sneak not working

It requires /skill sneak to function, and will not work with vanilla minecraft sneaking.
