    Okay your special key is gonna be HC__ . The first blank after HC is gonna be t, like you had above, then the second blank is the amount of numbers in your minecraft username. Please fix that, also your age in your profile and your age in this application are different, whys that?

    I can't find any information on your ban, how long ago was it? Also do you now have full control of the account? If possible change your password so your friend can't access your account.
