If you are untrained to use an item such as a sword as a ranger, attempting to hold it in your off hand will remove it and tell you that you are unable to use it. It wont place it in your inventory, it will delete it. However, sometimes I was able to swap hands and leave the game and reenter and the item would be in my inventory and offhand given that i had nothing in my main hand. That was not 100% consistent though, but the deletion was.
