Bug Party Roll items not recieved

As seen in the above screen shot 5 items were dropped from the boss, but only 3 of them were distributed to the party. Nobody got the tome fragment or the tiny soul shard.

This screenshot was taken a couple minutes after, while we were waiting for the boss to respawn so I had to scroll up to take it, they weren't simply further down in my chat..

There were 3 of us in the party, we were all voting need on all the drops, and nobody got those 2.

Unsure how or why this could happen, @Kashike - perhaps a specific string in the item name that could make it fail from giving it out?
