3. When my account was hacked i got banned on some servers i'm not sure all of them but here are the ones i do know. Duncsweb i was banned then unbanned after an appeal. Sippycraft i was banned while my account was hacked and i didnt appeal to it. us.oc.tc I was banned while i was hacked as well i think that might be all but i'm not sure sorry if this isn't detailed enough :/

Firstly you can only apply for one account. We do not allow alts. Other one was already whitelisted.
